Your Wellness Sources supports individuals 21 years and older with intellectual and developmental disabilities (IDD) to live self-sufficient everyday lives to the best of their abilities. We do this by helping individuals to have meaningful engagement in their communities.

Our Nursing Support Services cover an array of services that provide nursing consultation, health case management and delegation services based on the participant's need. Nursing Support Services DO NOT include direct nursing care services to the participant. Nursing Support services promote use of community resources and build on the participant's existing support network for positive health outcomes. Nursing Supports the needs of participants that will enable them to participate in the community and enhance each participant's quality of life.
Our Personal Support Services program specializes in providing customized services to participants with intellectual and developmental disabilities, along with their families. We assist participants to maintain independence, self- direction, self-determination, productivity, integration and participate in community inclusive activities, while also supporting their families.
All services provided through personal supports are to maintain a good quality of life for the participant while ensuring and supporting their safety and security, healthy lifestyle, spirituality, citizenship, and advocacy. We assist the participant to become connected within community networks to foster socialization, learning, innovation, and community. Our program support participants in meeting their needs that ranges from drop-in assistance, 24-hour supports-care. Our personal support services are provided in settings where the participant is living with their families, or living independently in private residences in the community.
Our Self-Directed services ensures flexibility to choose the supports that are right for participants and their family, including the staff, schedule of their choice, the technology, and expertise to support you every step of the way.
You can choose Self-Directed Services if you are eligible for Maryland DDA services and enrolled in the Home and Community-Based Services (HCBS) Waiver.
Choosing to Self-Direct your services with YWS means you are partnering with one of Maryland's leading providers, who can also provide integrated medical and behavioral health treatment services.
Our Supported Living program provide participants with personalized services to support their independent living in the community. These services are delivered according to the participant's need and personal interest as outlined in their person-centered plan.

Through our supported Living services our client will be able to live in a house or apartment of his or her choice located where he or she wants to live or live with other clients or participants of his or her choosing which do not include relatives, legal guardians, or legally responsible persons.
Our supported living program is designed to foster participants' nurturing relationships, full membership in the community, and work toward their long-range personal goals.
Our Community Living Enhanced Supports program provide services to participants in the community, who exhibits challenging behaviors, or have court ordered restrictions with intellectual and developmental maintenance skills that are related to activities of daily living. We provide participants with opportunities to develop their skills related to activities of daily living, instrumental activities of daily living, socialization, and safety (self and others) including but not limited to:
Our Community Living - Group Home program assist participants to develop living arrangements that meet their personal needs and desires, while allowing them to develop connections that enable them to become valued members of the community. Our program will support participants that need 24-hour supervision.
Our Community Living - Group Home program services are provided in a range of settings from homes owned/rented by the participant/family to apartments, homes and town homes leased by the agency in partnership with the participant.
Our Respite Care program provides short-term relief for family/primary caregivers of participants with Intellectual and Developmental Disabilities (IDD). Respite can be arranged for an afternoon or for several days or weeks. Care can be provided at the participants home or in the home of a Direct Support Professional.
