Mobile Primary Care Services at Assistant living or Senior Living Facilities
In-home primary care services for assistant living facilities, retirement communities, group homes, etc. We provide personalized care for health promotion, and chronic illness management that include fall risk, pain, dementia, and incontinence. Our providers are experienced in counseling and supporting your needs in advanced directives, end of life care, as well as Maryland State and Federal regulations for long-term and rehabilitation facilities. Our medical team consist of providers specialize in adult primary care, geriatric medicine, oncology, palliative care.

Our team of medical providers develop strong relationships with clients and facility’s nursing staff. We also provide patient education and nursing staff in-services as determined to improve patient care and well-being.

  • 60 minute new patient visit
  • Follow up visits
  • After Visit Summary at end of visit
  • Prevention and wellness
  • Urgent visits 5 days per week
  • Immunizations
  • Lab Services
  • Timely prescription refills
  • Electronic communication
  • Palliative/hospice care collaborations

Note:: Medicare Patients are required to give us permission to bill Medicare for chronic care management services provided to them. These services include care coordination, communication with patients and family over the phone, discussion with nurses or specialists, and medication management that occur for 20 minutes or more during a 30 day period.
